Coverage & Payment

Insurance & Fees

We are in-network with Blue Cross Blue Shield PPO and United Health Care, and we work with all other plans on an out-of-network basis. Here's everything you need to know about coverage, payment options, and how to verify your benefits.

Coverage Status

Insurance Coverage

Xceed Wellness is an in-network provider with Blue Cross Blue Shield PPO and United Health Care — we bill these plans directly on your behalf. For all other insurance plans, we are an out-of-network provider. Many of those patients still receive partial reimbursement through their plan using a superbill we provide after each visit.

What Is a Superbill?

For patients with insurance plans other than BCBS PPO or United Health Care, we provide a superbill upon request after every visit. A superbill is an itemized receipt containing all the diagnostic and procedure codes your insurer needs to process a reimbursement claim — simply submit it to your provider and they will reimburse you according to your out-of-network benefits.
